Hybrid prostheses.
A hybrid is a fixed full-arch prosthesis built as a metal framework carrying denture teeth in acrylic. It is repairable, kind to the opposing dentition, and it needs more maintenance than the alternatives.
What a hybrid is
A rigid framework, screwed to the implants, with denture teeth set in acrylic over it. The framework provides the strength and the passive fit; the acrylic and teeth provide the form, the aesthetics and the tissue-facing surface.
It reproduces the appearance of teeth and gingiva together, which suits cases where alveolar resorption has been substantial and the prosthesis has to replace tissue as well as teeth.
The name reflects that it combines fixed prosthodontic and removable prosthodontic principles — it is fixed to implants but constructed like a denture.
Compared with monolithic zirconia
A monolithic zirconia full-arch prosthesis is stronger, more stain-resistant, and does not wear at the occlusal surface. It is also harder on the opposing dentition, considerably more difficult to repair, and unforgiving if a fracture occurs.
A hybrid wears and stains over years and can chip a tooth — and each of those is repairable chairside or at the bench without remaking the prosthesis. Where the opposing arch is natural dentition or another prosthesis, the acrylic is kinder to it.
Neither is simply better. The choice depends on the opposing arch, the patient's function, how much tissue has to be replaced, and how maintenance will be handled.
Framework design
The framework carries the load and establishes the passive fit, so its dimension and rigidity matter more than anything else in the prosthesis. A framework that flexes transmits stress to the implants and cracks the acrylic over it.
It also has to be shaped so the acrylic is supported rather than bridging unsupported bulk — particularly at the distal extensions and around the screw access channels, which are where acrylic fractures start.
Adequate bulk of acrylic over the framework matters too. Where restorative space is tight, the acrylic thins over the metal and fractures.
Maintenance expectations
Denture teeth wear, and on a hybrid opposing natural dentition they wear faster. Over years that changes the occlusal scheme and reduces the vertical dimension, which is a planned replacement rather than a failure.
Acrylic stains and can develop a rough surface that accumulates plaque. Teeth can debond. Screws loosen. None of these is unusual and all are manageable, but the patient should be told at the outset that this is a prosthesis requiring ongoing care.
Retrievability is therefore essential, which means screw retention.
Hygiene access
The tissue surface should be convex and highly polished, with genuine space beneath for an interdental brush. A concave intaglio that traps debris produces peri-implant inflammation regardless of how well the prosthesis fits.
This is the single most common design compromise on hybrids, because closing the space looks better and the consequences appear later.
Show the patient specifically how to clean beneath it and have them demonstrate before they leave.
Records
As for any full-arch case: a verified implant record, the opposing arch, an established vertical dimension and jaw relation, and a provisional or diagnostic set-up establishing tooth position.
Component detail for every implant. The prosthesis type intended, the material for the teeth, and the tooth shade and mould.
Where a provisional has been worn and adjusted successfully, record it — it is what the definitive prosthesis should reproduce.
