Dentist Resources

Everything a practice needs to send cases well — the forms, the guides, and the answers to the questions that otherwise become phone calls.

Incoming dental cases being inspected at a laboratory reception bench

New to the laboratory? The new client guide covers opening an account, getting set up with forms and a scanner connection, and what the first few cases look like.

Prescription forms are on the Rx forms page, with both slips available to download and printed pads available on request.

For pricing, request the current list through the fee guide request.

Getting a case right first time

The preparation guides set out what to check at the chair before a record is taken, by case type. Shade matching covers taking and transmitting a shade that survives the journey to the bench.

The material guide compares everything on the prescription — fixed, removable and appliances — in one place.

For digital submission specifically, the digital case checklist is the short version.

What the laboratory needs from every case

Practice and doctor identification with a reachable number. Patient identifier. Tooth numbers marked on the design chart. The material stated explicitly. The shade with the guide named. The margin type prepared. The antagonist. The date you need it.

For implant work, add the system, product line, platform diameter, connection type and intended retention. For anterior work, add photographs.

Anything unusual belongs there too — a short preparation, clearance accepted as tight, a margin uncertain on one surface. Those change what we do and they prevent a call.

Hours and contact

The laboratory is open Monday to Friday, 9:00 am to 5:00 pm, at 5349 Imperial Street in Burnaby.

Cases move by UPS and Purolator, and Fairmont works with dental offices throughout British Columbia and across Canada.

Patients attend only by appointment for shade matching arranged through their practice — the laboratory is not open to the public.

When you want to talk to someone

Advising on demanding cases is part of the service rather than an extra. Ask a technician covers what the technical resource is for and how to use it.

Common questions are answered on the FAQ page, and the Knowledge Centre carries longer technical articles for the questions that need more than a paragraph.

The laboratory is open Monday to Friday, 9:00 am to 5:00 pm.

One of the biggest advantages of working with Fairmont is knowing that, when we have a question, an experienced technician is available to help. Their collaborative approach makes challenging restorative cases easier to manage.
